The Care Coordinator assists with health care planning of client needs
- Monitor health status and progress
- Serve as a liaison between patient/family, service providers and insurance carriers
- Contain costs through monitoring of care and elimination of redundant or unnecessary treatment/procedures/DME
- Provide effective discharge planning for reintegration into the community as appropriate
- Formulate and implement a plan of care focusing on current and future needs with available resources
- Identify short and long term care needs
- Coordination with Special Needs Trusts
- Assistance with coordination of disability benefits
